Synapse’s Lifestyle Support Service provides services to assist people to live independently in the community through developing and maintaining independent living skills.
Model of Support
The Association operates within a formalised Model of Practice which includes, but is not limited to:
- Positive Behaviour Support,
- Client-Centred Planning,
- Strengths-Based Approach,
- Behaviour Consultancy.
What do we do?
The services we provide are varied and include, but are not limited to:
- Community Access – supporting clients to access vocational, leisure and social activities, developing social networks and attending appointments.
- Home Management – supporting clients to pay bills, budget, prepare meals, shopping, complete household tasks.
- Personal Care – overnight support, mobility transfers, showering, toileting, medication management
Eligibility criteria:
- The Client must have an acquired brain injury or behaviours that challenge our understanding
- The client must have a funding package or be compensable e.g. Government funding, funding from insurance, or private funding
- Live in the greater Brisbane area
